Online consultations information and frequently asked questions

OUR PATIENT CARE ADVISORS WILL TRIAGE YOUR CALL AND NAVIGATE YOU TO THE MOST APPROPRIATE CLINICIAN FOR YOUR PROBLEM. WE ASK FOR YOUR COOPERATION WHEN SPEAKING TO THE ADVISORS AS THEY ARE ACTING ON THE INSTRUCTIONS OF THE GP PARTNERS.

We are currently working on a  triage model, which means that requests for appointments, wherever possible, can be submitted via an online form called  an eConsult,  which you can find below. Our staff are happy to help patients complete these forms over the telephone, to show them how to use the system. We also understand that not all patients have access to a computer and in those cases we can help with requests over the telephone too.

When we are at capacity the option to submit an eConsult to request an appointment form will be temporarily suspended until 08:00am the next working day, however patients will still be able to submit a self help or pharmacy advice form. 

Your GP or nurse will review your problem and will then, depending on your needs offer you advice via text, offer you a telephone call, offer you a video call, ask to see you in person (face to face) or navigate you to other services. Please use the eConsult forms below, just click on "Get Started". You don't need a username or password in order to fill out a request form.

eConsults can be submitted Monday to Friday from 08:00-18:30pm and are not available at weekends.

For urgent matters between the hours of 18:30pm and 08:00am patients are advised to contact the NHS Service 111 and for medical emergencies only, ring 999.

Please note that when the surgery is closed for protected learning times , patients will not be able to submit an eConsult for clinical advice from 1pm onwards. The PLT dates are listed under our surgery opening times tab.
